Agreement with personal data processing for marketing purposes

I agree that WT WINTECH a.s., with its registered office at Seifertova 2834/33, Přerov, postal code 750 02, Identification No. (IČ): 65138708, Tax Identification No. (DIČ): CZ65138708 (hereinafter the “Trustee”) in the sense of the Regulation of the European Parliament and of the Council EU 2016/679 processes the following personal data:

• Name and surname;
• E-mail (hereinafter the “Personal Data”)

1. The Trustee shall process the above-mentioned data for marketing purposes, i.e. mailing the Trustee’s newsletters to a person who gives this consent. This consent is given for a period of ...... (according to the products type, we considered a period of 3 – 5 years as a defendable period) years.  The person providing such consent shall take into account, that the Trustee shall process the above-mentioned personal data for the period of the validity of the consent.
2. Only the Trustee shall process the Personal Data, or as the case may be, a person with which the Trustee duly concluded a processing agreement.
3. The consent may be withdrawn at any time for example by sending an e-mail or letter to the contact data of company, Seifertova 2834/33, Přerov, postal code 750 02, eshop@wintech.cz.
4. WT WINTECH a.s. shall inform its customer that they are entitled:
   1. to obtain a confirmation from the Trustee whether the personal data relating to them are or are not processed, and if it is so, they have the right to obtain access to such personal data and to the following information: processing purposes, categories of the relevant personal data, recipients or categories of recipients to whom the personal data were or will be made available, in particular recipients in third countries or in international organizations, planned period for which the personal data will be deposited or if such period cannot be determined, the criteria used for the determination of such period,  the existence of the right to request from the Trustee a correction or deletion of the personal data concerning the data subject  or limitation of their processing or to raise an objection against such processing, the right to lodge a complaint at the supervisory authority;
   2. that the Trustee corrects without undue delay imprecise personal data  to which they relate. Taking into account the purposes of the processing, they shall be entitled to complete incomplete personal data even by providing an additional statement;
   3. that the Trustee deletes without undue delay personal data that relate to them and the Trustee shall be obliged to delete personal data without undue delay if one of the following reasons exists: personal data are no longer necessary for the purposes for which they were collected or otherwise processed, the customer withdraws its consent and there is no other legal title for processing thereof, personal data were processed unlawfully, personal data must be deleted to fulfil a legal obligation set forth by the acquis communautaire or the laws of the member state which relate to the Trustee;
   4. that the Trustee restricts processing of their personal data if they deny accuracy thereof for the period of verification of such fact by the Trustee, the processing is unlawful and they ask the Trustee for their limited processing instead of deletion,
      the data are no longer necessary for the purposes of the processing, but they need them to determine, perform or defend legal claims;
   5. that the Trustee provides at their request their personal data to another Trustee appointed by them;
   6. to raise an objection with the Trustee against processing of my personal data; or
   7. in case of any doubts whether the Trustee processes their personal data pursuant to the above-mentioned legal regulations, to contact both the Trustee, and the Personal Data Protection Authority.

Consent to Personal Data Processing for the Purposes of Registration

I agree that WT WINTECH a.s., with its registered office at Seifertova 2834/33, Přerov, postal code 750 02, Identification No. (IČ): 65138708, Tax Identification No. (DIČ): CZ65138708 (hereinafter the “Trustee”) processes the following personal data pursuant to the Regulation of the European Parliament and of the Council EU 2016/679:

• Name and surname;
• Address;
• Telephone number;
• E-mail; and
• in case of persons doing business, also Identification No. (IČ) and Tax Identification No. (DIČ) (hereinafter the “Personal Data”)

1. The Trustee shall process the above-mentioned data for the purposes of registration at the Trustee’s website/e-shop. This consent is given for a period of .... years. The person giving such consent shall take into account that the Trustee shall process the above-mentioned personal data throughout the period of the validity of the consent.
2. Personal data shall only be processed by the Trustee or, as the case may be, by a person with whom the Trustee duly concluded a processing agreement.
3. The consent may be withdrawn at any time for example by sending an e-mail or letter to
   the contact data of the company, Seifertova 2834/33, Přerov, postal code 750 02, eshop@wintech.cz.
4. WT WINTECH a.s. shall inform its customers that they are entitled:
   1. to obtain a confirmation from the Trustee whether the personal data that relate to them are or are not processed, and if it is so, they are entitled to obtain access to such personal data and to the following information:  processing purposes, categories of the relevant personal data, recipients or recipients categories to whom personal data were or will be available, in particular recipients in third countries or in international organizations, planned period for which personal data will be deposited or, if it is impossible to determine such period, the criteria used to determine such period, existence of the right to request from the Trustee a correction or deletion of personal data concerning the data subject or limitation of the processing of such data and to raise an objection against such processing, or the right to lodge a complaint with
      a supervisory authority; 
   2. that the Trustee corrects without undue delay imprecise personal data that relate to them. Taking into account the processing purposes, they shall be entitled to complete incomplete personal data including by providing an additional statement; 
   3. that the Trustee deletes without undue delay personal data that relate to them and the Trustee shall be obliged to delete personal data without undue delay for one of the following reasons: personal data are no longer necessary for the purposes for which they were collected or otherwise processed, the customer withdrew its consent and no other legal title exists to process such data, personal data were processed unlawfully, personal data must be deleted to fulfil a legal obligation set forth by the acquis communautaire or a member state which relates to the Trustee;
   4. that the Trustee restricts processing of their personal data if they deny their accuracy for the period of verification of such fact by the Trustee, the processing is unlawful and they ask the Trustee, in place of the deletion, for restricted processing of such data, the data are no longer necessary for the purposes of the processing but they need them to determine, exercise or defend legal claims;
   5. that the Trustee provides their personal data at their request to another Trustee appointed by them;
   6. to raise an objection with the Trustee against processing of my personal data; or
   7. in the case of any doubts whether the Trustee processes their personal data pursuant to the above-mentioned legal regulations, to contact both the Trustee and the Personal Data Protection Office.
